Do you mean a Risk Assessment? That is commonly requested by large organizations - I have one on my free resources page (link in my signature) in a PDF form that you can use. Just adapt it for your own use.

Shannon! You are a life saver!! and an extremely talented artist!! I have been painting for 5 years and hope to continue and keep getting better!! Your resources have really really helped me and my not so business savy brain!! I have just bought your face charts and cant wait to check them out when they get to me all the way in lil New Zealand!! the world needs more people like you!! Thank you again!! Would love to see you one day in NZ!
