Need Sample Please ......

Anyone have a sample of a model release form ? I'm not sure what to put in a face or body painting contract...... Please send me a sample I'm new to this .......... Need help.

Shannon Fennell has a TON of resources on her blog, including a model release form. It is accesible through the links in her signature.
